Important:
The signer must personally appear before the notary and must be able to communicate
and participate in the notarization. If the signer cannot communicate or appears
unaware of what is taking place, the notarization may need to be postponed or declined.

directly in the patient's hospital room when permitted by the facility.

What if the signer is weak or bedridden?

Being bedridden does not automatically prevent a notarization. A bedside notarization
may be possible as long as the signer can personally participate, communicate with the
notary, and indicate a willingness to sign.

What identification is required for a hospital notarization?

A driver's license, state identification card, passport, or another acceptable form of
identification may be used depending on the circumstances. If the patient does not
have their usual identification available, call before scheduling so we can discuss
what identification is available.

Yes. Independent witnesses may be available by advance arrangement.

Can a family member serve as a witness?

Witness requirements depend on the document and the circumstances. Because those
requirements can vary, it is best to discuss the document before the appointment.
Independent witnesses can often be arranged when needed.

What if the patient is heavily medicated, confused, or unable to communicate?

The notary must be able to interact directly with the signer and determine whether the
signer is participating knowingly and voluntarily. If the patient cannot communicate,
appears unaware of what is occurring, or cannot indicate a willingness to sign, the
notarization may need to be postponed or declined.

How quickly can you come to a Pittsburgh hospital?

Same-day and emergency hospital appointments may be available. Availability depends
on the hospital location, time of day, document requirements, and whether witnesses are needed.

Do the documents need to be completed before you arrive?
